Athletic Physicals

Any student participating in a sport needs a ATHLETIC PHYSICAL REPORT FORM filled out if there is a possibility that the student will participate in any of the after-school Athletic Programs. This form must be completed each year after July 1. We recommend taking care of this when having summer doctor appointments.

Each school sport requires 2 eligibility documents:

1. A physician signed Archdiocesan Sport Physical (“Pre-Participation Physical Evaluation” Form) 
   This is located on the website and expires a year after the physician signature. A sports physical is required for all students who plan to participate in the athletic program. It is recommended that the physical be completed during the summer so that it remains valid for the entire school year. Please note that the diocese updates the required forms annually.

2. Athletic Handbook Acknowledgment Process
   We kindly request that you thoroughly review the Athletic Handbook alongside your athlete. This handbook contains essential information regarding the rules, regulations, and expectations for participation in our athletic programs.

   1. Steps to Complete:
      Read the Athletic Handbook and return to the Athletic Department with all the needed signatures.
